Needed at Time of Registration
Pre-Kindergarten and Kindergarten applicants need to bring a copy
of the following documents:

Birth Certificate

Baptismal Certificate
3.
Updated Immunization
Record
4.
Social Security Card
First grade applicants need to bring in the same documents as
stated above plus a Kindergarten Report Card.
Second through Eighth grade applicants need to bring a
copy of the following documents.

Birth Certificate

Baptismal Certificate
3.
First Communion
Certificate
4.
Updated Immunization
Records
5.
Social Security Card

Current Report Card
7.
All previous Report
Cards
8.
All Standardized Test
